Review and Agree to the Following Policies and Disclosures Before Purchase.
Fees: The client agrees to purchase a Coaching Session at the price stated on the order form. If a payment plan is selected, then the first payment is due upon purchase.
Missed Payments: If payments are not completed after three attempts to collect, then the client’s access to the product will be revoked until the outstanding balance is paid in full.
Refund and Expiration Policy: Coaching sessions are non-refundable. There are no partial refunds on unused coaching sessions from a package. Unused coaching sessions expire 6 months after purchase.
Scheduling: Clients schedule coaching sessions using Acuity Online Scheduling System. Samantha is typically available for client appointments on Mondays, Tuesdays, and some Wednesdays between the hours of 8:00 AM and 7:00 PM EST. Coaching sessions are 45-minutes long. These sessions are virtual and will be conducted online via Zoom.
Cancellation and Rescheduling: Clients may cancel or reschedule a session up to 24 hours in advance. If an appointment is cancelled less than 24 hours in advance, that session will count towards the client’s overall purchase. Please contact Samantha directly if there is an extenuating circumstance.
No Show Appointments: Samantha will wait up to 15 minutes from the start time of all coaching sessions on Zoom. If the client fails to join the call, that is considered a "no show appointment." These appointments are non-refundable and count towards the total number of sessions purchased.
Confidentiality: Samantha will maintain the confidentiality of all information shared.
Disclosures: The client understands that Samantha Calonita is not a physician, psychiatrist, psychotherapist, licensed therapist, or clinical social worker, and is not trained in nor does she diagnose or treat any psychological or medical condition. If an issue should arise that is best addressed by a licensed medical or psychological professional, she will recommend the client contact them for further assistance. The client also acknowledges and agrees that they have free will and that they are fully responsible for all of their thoughts, feelings, and decisions and for any actions they may take on those thoughts, feelings, and decisions. The client agrees that they will not hold Samantha or her business entities responsible for any of their own actions, or the consequences, committed by them at any time and that they will not institute any lawsuit or legal proceeding against Samantha Calonita relating to events occurring or actions taken in their life.
